The range of services provided to seniors is listed below from least restrictive to most restrictive:

Elder Adult Substance Abuse Prevention   (back to top)

The Get Connected Program is a three-session presentation offered throughout Pinellas County to groups of seniors with referrals to intervention programs when needed. The Get Connected prevention program also speaks with seniors to promote Healthy Living and to educate them on the awareness of abuse/misuse of medications, alcohol or other substances. Through this community based presentation, the senior population is able to connect with each other and reach out to a natural support system.


Older Americans Act Title III-B Counseling    (back to top)

In-home or in-office Mental Health (for persons with severe and persistent mental illness, provided by State of Florida Licensed Mental Health Counselor) or Gerontological Counseling (for persons experiencing emotional/situational difficulties utilizing a case management approach, provided by Masters Level Counselors and Bachelor level [or equivalent] volunteers).

Educational talks are provided to persons attending congregate dining sites to inform participants of mental health issues, coping skills, etc. This service is provided at various senior service sites throughout Pinellas County.


Community Care for the Elderly Case Management Programs   (back to top)

Provision of case management services and service authorization following assessment to persons throughout Pinellas County who are at risk of nursing home placement.

• CCE Programs are the largest, serving 200 persons annually each, providing numerous services so that persons will remain in their location of choice.

• Home Care for the Elderly serves 10-15 persons annually who require 24-hour supervision by caregiver(s) and provide financial assistance plus supplies and respite reimbursements when needed.

• Alzheimer’s Disease Initiative serves persons 20 and above diagnosed with memory loss causing them to be at risk for nursing home placement; program serves 40 persons annually.

• Aged and Disabled Medicaid Waiver serves persons who are at risk for nursing home placement and are financially and functionally eligible for Medicaid; program serves 250 persons annually.

Assisted Living Medicaid Waiver serves persons at risk of nursing home placement and residing in assisted living facilities throughout Pinellas County; program serves 180-220 annually, and this is generally the frailest group with the most intense services of all.

 

Additionally, Suncoast Senior Support Programs may link with other Suncoast programs as needed, including: Adult Outpatient services and Adult Medical services for provision of medications, day treatment services, homeless services, the for emergency services and substance abuse intervention programs.
